This permanent full-time position emphasizes collaboration within an integrated healthcare team. You will assess, diagnose, and treat patients while delivering culturally safe care, especially for Indigenous communities. In this role, you'll implement and support specialty clinics, conduct home visits, and educate clients on health issues to promote wellness.

Key Responsibilities:

• Assess and diagnose health issues for patients

• Develop specialty clinics targeting community health needs

• Act as a health advocate for clients

• Ensure accurate documentation of client visits

• Engage in community health promotion programs

Requirements:

• Active registration as a Nurse (Extended Class) in Ontario

• 3-5 years of nursing experience in diverse settings

• Skills in addressing mental health and addiction issues

• Excellent communication and organizational abilities

• Valid Driver’s Licence and reliable vehicle access
