As a full-time Registered Practical Nurse (RPN) at Providence Care, you will engage with clients exhibiting responsive behaviours linked to dementia, addictions, and other neurological conditions. This position focuses on enhancing quality of life through effective transitions and supportive management strategies. Key Responsibilities:
• Provide direct client care alongside MRT RN and PSWs
• Collaborate with Seniors/Geriatric Mental Health Services
• Utilize screening and assessment tools for client evaluations
• Develop and implement tailored Collaborative Care Plans
• Work across multiple LTCHs in the assigned region
Requirements:
• Current unrestricted RPN registration in Ontario
• RPN Diploma with medication certificate or full practice
• Minimum one year experience with older adults
• Strong communication and teamwork skills
• Satisfactory criminal record screening
