As a Clinical Planner with Para Med, you will play a crucial role in evaluating and establishing care plans for new patients within your assigned district. This full-time role requires at least two years of nursing experience, along with strong critical thinking and communication skills. You will ensure that care delivery respects cultural needs and meets best practice standards.
Key Responsibilities:
• Conduct assessments for new patients to determine care needs
• Complete safety assessments of living environments
• Implement initial personal care services for patients
• Advise on scheduling parameters for ongoing visits
• Communicate Para Med policies to patients and families
Requirements:
• RPN/LPN registration in good standing
• Minimum two years nursing experience required
• Knowledge of applicable healthcare legislation
• Strong organizational and communication skills
• Access to a personal vehicle mandatory
