Dedicated Coordinator for Service Planning

Location: Brockville

Transform lives as a Dedicated Service Planning Coordinator with Developmental Services of Leeds and Grenville. This temporary part-time position emphasizes family-centered approaches for children with complex needs.

As part of the Children's Services Team, you will support families in creating coordinated plans tailored to their unique requirements.

Your role entails engaging with multiple agencies to ensure seamless service delivery and helping families manage their children’s development effectively. Building strong connections with families is at the heart of this position.

Key Responsibilities:

• Complete referral processes to establish client needs

• Create comprehensive support plans based on individual assessments

• Lead case planning discussions with relevant stakeholders

• Provide training and resources to parents

• Assist with applications for financial support

Requirements:

• 4-year undergraduate degree in a relevant field

• At least 3 years’ experience in family services

• Proficiency with community resources

• Strong interpersonal skills and empathy

• Bilingualism is a plus
