Access to Care Social Worker

Position: Access to Care Social Worker Role
Enhance patient care as a temporary full-time Social Worker at Providence Care, focusing on access to care and transitions. Apply your MSW knowledge to improve treatment goals for clients and families.

This role involves providing case management, patient education, and supportive counseling to assist individuals during their healthcare journey. You'll lead projects to optimize discharge processes and ensure clients receive comprehensive support through community resources.

Key Responsibilities:

• Perform psychosocial assessments and crisis interventions

• Use evidence-based strategies for client support

• Advocate for family support team conferences

• Collaborate on discharge planning initiatives

• Adjust treatment plans based on ongoing evaluations

Requirements:

• Master of Social Work (MSW) from a recognized institution

• Current registration with Ontario College of Social Workers

• Experience in a hospital setting preferred

• Knowledge of electronic patient record systems is a plus

• Strong community and advocacy skills
