Make a difference as a Service Planning Coordinator with Developmental Services of Leeds and Grenville. This temporary part-time role focuses on coordinating services for children and youth with complex special needs.

Join the Children’s Services Team at DSLG, where you will support families by creating individualized service plans and connecting them with appropriate resources. With over three years of relevant experience and extensive knowledge of best practices for developmental disabilities, you will play a crucial role in guiding youth and their families through service delivery as you monitor their progress.

Key Responsibilities:

• Conduct intake assessments for service eligibility

• Develop tailored support plans based on unique needs

• Facilitate case planning meetings with multiple agencies

• Provide family counseling and mediator training

• Identify service gaps and create specialized programs

Requirements:

• Bachelor’s degree in Humanities or related field

• Three years of experience with children’s services

• Knowledge of community service standards

• Strong communication and interpersonal skills

• Valid Ontario driver's license and reliable vehicle
