Psychosocial Support Social Worker Position

Providence Care is seeking a dedicated Social Worker to provide psychosocial assessments and support in a full-time capacity. Focus on patient recovery, education, and effective community transitions in this impactful role.

In the role, the Social Worker will report directly to the designated manager and will be responsible for case management, discharge planning, and family education. You will implement evidence-based treatment strategies and evaluate outcomes throughout the treatment process, ensuring alignment with ethical standards. Strong community links are essential for effective patient support.

Key Responsibilities:

• Conduct assessments using evidence-based methods

• Provide supportive counselling during crises

• Develop personalized treatment goals with clients

• Coordinate community resources for transitions

• Manage discharge plans and client expectations

Requirements:

• Master of Social Work (MSW) degree

• Current registration with Ontario College of Social Workers

• Experience in hospital Social Work (two years preferred)

• Proficiency in electronic patient record systems preferred

• Proven advocacy skills and community outreach ability
