Position: Case Worker for Community Support Services

Empower individuals as a Case Worker in the Community Outreach team, providing critical support to clients navigating mental health and addiction challenges.

Your role will focus on delivering personalized care while mobilizing resources and support networks effectively.

This full-time temporary position operates within community policing frameworks to assist at-risk individuals. You will combine your problem-solving skills and knowledge of recovery philosophy to create a supportive environment for clients and their families. Effective communication and decision-making will drive your work, ensuring clients receive the best possible outcomes.

Key Responsibilities:

• Develop and implement individualized support plans

• Work collaboratively with community partners for resource integration

• Provide immediate crisis intervention as needed

• Gather, evaluate, and utilize client data effectively

• Ensure compliance with health and safety regulations

Requirements:

• Experience in mental health and addiction (2 years)

• Relevant diploma in a health-related field; degree preferred

• Valid First Aid and CPR certifications

• Hold a valid Ontario Driver's Licence

• Clear Vulnerable Sector police check required
