Position: Social Worker for Integrated Care Solutions
Become a Social Worker with Bayshore Health Care and enrich the lives of clients in Kingston through community support and tailored care plans. This part-time position emphasizes person-centered approaches for optimal client independence.

In this role, you’ll assess clients' needs and facilitate community resource access. Work collaboratively with families and other professionals to enhance the psychosocial treatment process and ensure that clients receive the best possible care tailored to their situations.

Key Responsibilities:

• Assess adjustments to medical and social challenges

• Develop and modify individualized treatment plans

• Advise on community resources and assist with referrals

• Maintain a dynamic, organized caseload

• Maintain communication with support networks and service providers

Requirements:

• Active member of Ontario College of Social Workers

• Preferred two years of therapy experience

• Excellent English communication skills;
French considered an asset

• Current driver’s license and reliable vehicle preferred

• CPR certification necessary
