About the Role and our Ideal Candidate

You will need to registered with Social Work England.

The role will require you to:

- Facilitate effective assessment and support planning in accordance to the Care Act 2014.

- Be responsible for undertaking reviews of adults drawing on care and support, and supporting them and their needs as required, within the statutory timescales.

- To care manage a number of cases, including undertaking safeguarding investigations. The work will vary with different levels of complexity and risk.

- Achieve effective communications with colleagues in other departments within health and social care to ensure efficient and person centred multidisciplinary support planning.

- Ensure that onward required referrals to the correct service are made.

- Consider the correct funding path (e.g. ASC, CHC, Section 117, joint social care and health funding).

- Make effective use of resources through care planning, signposting to other services, or providing individuals with information and advice.

About Us

We have various opportunities for you to join Kingston Adult Social Care as a Newly Qualified Social Worker. Adult Social Care serves the adults within the borough with care and support needs.

We have a number of different teams who work with adults as part of our statutory duties. This includes our hospital discharge and access team, community learning disability team, mental health social care team, and our 3 locality hubs (supporting older adults and people with physical disabilities).

We have an established and supportive Assessed and Supported Year in Employment (ASYE) programme which includes regular supervision, reduced caseload, protected time for professional development, professional development plan discussions, action learning shops and workshops, and regular reviews for constructive feedback to support continuous development.
