Job Title: Social Worker - Mental Health - Kingston Upon Thames
Pay Rate: £31.31 per hour
Liquid Personnel is currently seeking an experienced Adult Social Worker to join the Mental Health Team of our client. We are looking for a qualified social worker with experience in mental health to review supported living placements, with the majority of reviews conducted jointly with Health partners.
Responsibilities
- Evaluate patients' mental health status, identifying needs, strengths, and areas for intervention.
- Collaborate with the healthcare team to create individualized treatment plans that address patients' mental health needs and goals.
- Respond to mental health crises, providing immediate support and intervention to stabilize patients.
- Attend interdisciplinary team meetings and case conferences to discuss patient care and collaborate with other healthcare professionals.

Qualifications and Experience
- Social Work England registration
- Eligible to work in the UK
- Qualification in Social Work at degree level
