Description

Indigenous Chronic Kidney Disease (CKD) Navigator – Full-Time (Kingston)

Support Indigenous patients and families as they navigate the kidney care journey—from early CKD screening to advanced therapies. This role provides culturally safe navigation, advocacy, and care coordination, partners with Indigenous communities, and advances regional and provincial Indigenous renal health priorities.

  • Champion early detection:
    Help organize Indigenous CKD risk screening clinics, support tailored referral pathways, and share early screening guidance with partners.
  • Guide & advocate:
    Provide culturally safe navigation, advocacy, and coordination of care across the renal continuum; act as a liaison between patients, families/caregivers, and clinical teams.
  • Advance strategic priorities:
    Contribute to regional/provincial renal and Indigenous health priorities and participate in relevant communities of practice/leadership forums.
  • Promote cultural safety:
    Support cultural competency education and embed Indigenous traditions/spirituality/holistic practices into care delivery.
  • Build partnerships & report:
    Strengthen relationships with First Nations, Inuit, Métis, and urban Indigenous partners; track and document program activity aligned with OCAP principles.
