Responsibilities

  • Provide high‑quality, skilled nursing care and support the Community Nurse Team Leader in planning, implementation, and evaluation of holistic patient needs.
  • Assist the District Nurse and Team Leader in coordinating the nursing team to ensure effective caseload and care management.
  • Coordinate and supervise nursing team duties during the absence of the District Nurse Team Leader.
  • Undertake initial assessments, reviews, planning, implementation, monitoring and evaluation of individual care programmes.
  • Deliver a high‑quality service to clients and carers independently, planning, implementing and evaluating care programmes.
  • Assist and carry out implementation of local and national initiatives, health promotion, national service frameworks and NICE guidance.
  • Ensure infection prevention and control measures are followed and maintain patient records in line with Trust policy.
  • Liaise with agencies within and outside the health service, act as an advocate for clients, and participate in case conferences and multi‑agency meetings.
  • Identify vulnerable adults and respond in line with safeguarding policy; provide health promotion and participate in related activities.
  • Assist in the assessment, supply, review and safety of equipment used to support nursing care within clients' homes, and contribute to the Clinical Governance Action plan for Community Nursing.
Qualifications
  • Nursing at Level5 or evidence of a back‑to‑nursing programme.
  • Post‑registration experience in the community setting (minimum1year).
  • Experience in palliative care, care of the older person, orthopaedics, or community services.
  • Valid registration with the NMC (RN1, RN2) and recognised qualifications in Health Promotion, Diabetes, Continence, Palliative care, Medical/Surgical, or Nursing.
Other Requirements

The post requires a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order)1975.
