• Provide direct resident care services in accordance with individualized service plans (ISP)
• Administer medications in compliance with all standards of practice, state regulations, and community policies
• Complete all required documentation, reporting, and follow-up accurately and timely
• Perform resident assessments as required under the direction of the Director of Assisted Living
• Monitor residents for changes in condition and report concerns promptly to leadership
• Document observations, care provided, and communication with families or responsible parties each shift
• Maintain and update individualized care plans for assigned residents
• Support a positive, respectful environment for residents, families, and team members
• Perform additional duties as assigned

• Reports to the Director of Assisted Living and/or Director of Memory Care
• Expected to work independently and exercise sound clinical judgment
• May assist with training, supervision, and evaluation of care staff
• May participate in performance reviews and disciplinary processes as needed

• High school diploma or GED required
• Active Licensed Practical Nurse (LPN) license in the state of Tennessee required
• Current CPR certification required
• Minimum of 1 year of related experience preferred
• Strong clinical, communication, and documentation skills

Physical Requirements:
Must be able to sit, stand, walk, bend, reach, and perform physical tasks throughout the shift. Must be able to lift up to 25 pounds unassisted and assist with resident transfers using proper equipment.
