Clinicians of Nurse Practitioners Services of Texas provide care to nursing home residents enrolled in an Institutional Special Needs Plan called Texas Independence Health Plan. Provides primary care and acute care management for patients in 4 nursing homes in collaboration with an RN Care Coordinator. The emphasis is on proactive and preemptive care through general health management, increased availability of on‑site clinicians, and early identification of changes in condition to avoid unnecessary hospitalizations and ED visits.

The NP establishes individualized care goals for each member and facilitates communication between members, families, providers, and facility staff, and establishes advance care preferences with each member. We strive to provide education about disease progression, address problems sooner in the disease process, and manage members in the skilled nursing facility whenever possible.

Locations:
Robstown, Kingsville, San Diego, and Alice, TX.

• Master's Degree in Nursing (MSN)
• Texas State licensure as a Nurse Practitioner
• Current certification as a Nurse Practitioner
• DEA Prescriptive License
• Ability to work independently and as part of a team
