Summary- The Registered Nurse (RN) operates as a key member of the health care team using the nursing process, clinical skills and leadership to direct, provide and evaluate age/population specific patient care according to the department’s scope of service. Analyze patient health problems and needs, develop and implement nursing care plans, and maintain medical records.
- Monitor, record, and report symptoms and changes in patients’ condition.
- Collect patient data; order, interpret, and evaluate diagnostic tests to assist in determining the plan of care.
- Identify expected outcomes and implement/modifying interventions identified in the patient plan of care.
- Evaluate the patient’s progress toward outcomes and modify patient treatment plans as indicated by patient response and condition.
- Prepare patients for, and assist with, examinations and treatments.
- Accurately prepare and administer medications according to policies and procedures.
- Document the nursing process in nursing assessment, nursing care plan (NCP), and nursing notes as outlined in policy and procedure. Diagnosis is valid for patient; interventions are appropriate and outcomes are evaluated. Changes in the plan of care are made as the patient progresses.
- Systematically evaluate the quality and effectiveness of nursing practice.
- Identify practices to improve job or department performance and monitor job and department activities.
- Participate in department CQI, including:
- Using results to initiate changes in practice and system.
- Acquiring and maintaining current knowledge in nursing practice.
- Participating in ongoing educational activities, seeking experiences to maintain skills, and consistently seeking educational tools to broaden knowledge base of various age groups, diseases, and best practices.
- Completing mandatory training each year, plus additional 16 minimal hours for staff development and remaining up‑to‑date on new equipment, policies, and procedures.
- Contribute to the professional development of peers, colleagues, and others by:
- Sharing knowledge with colleagues; providing constructive feedback.
- Directing and supervising less skilled nursing or health care personnel; possibly serving as a charge nurse or preceptor after attending the preceptor workshop.
- Adhering to unit-specific standards of patient interaction and customer service.
- Holding staff members accountable for results, ensuring adequate adherence to policies and procedures, and informing and training new team members of procedures and policies.
- Make decisions and take actions on behalf of patients in an ethical manner.
- Maintain confidentiality, act as a patient advocate, and deliver care in a non‑judgmental and non‑discriminatory manner that is sensitive to patient diversity.
- Collaborate with patient, significant others, and health care providers in providing care, including:
- Consulting and coordinating with health care team members to assess, plan, implement, and evaluate patient care plans.
- Making referrals for continuity of care.
- Educating patients, families and other groups on topics such as health education, disease prevention, and health improvement programs.
- Involving multidisciplinary teams as needed and documenting this involvement.
- Informing patients and family members of actions being taken, status of activities, estimated time involved, and reason for changes or delays.
- Use research findings in practice by:
- Using interventions substantiated by research.
- Participating in research as appropriate to the nurse’s position.
- Participating in improvement/redesign processes and implementing findings from departmental and performance improvement activities into daily tasks.
- Consider factors related to safety, effectiveness, and cost in planning and delivering patient care by:
- Evaluating factors related to safety, effectiveness, and cost when two or more practice options would result in the same outcomes.
