Position Summary:
The RN Case Manager is responsible for coordinating and managing patient care activities to achieve high-quality, cost-effective outcomes across the continuum of care. This includes patient assessment, discharge planning, utilization review, and collaboration with interdisciplinary teams to ensure timely and appropriate patient transitions. The RN Case Manager promotes patient advocacy and works to optimize clinical and financial outcomes through effective resource management.
Key Responsibilities:
• Perform comprehensive assessments to identify patient care needs, discharge plans, and post-acute requirements.
• Coordinate and facilitate safe, timely patient transitions through the continuum of care.
• Collaborate with physicians, nursing, therapy, and social services to develop and implement individualized care plans.
• Conduct concurrent review and utilization management to ensure appropriate levels of care.
• Monitor patient progress, identify barriers to discharge, and intervene as necessary.
• Educate patients and families regarding available community resources, discharge instructions, and care options.
• Maintain accurate and timely documentation in the EPIC electronic health record (EHR).
• Adhere to hospital policies, accreditation standards, and state/federal regulations related to case management and discharge planning.
• Participate in performance improvement initiatives and multidisciplinary team meetings.
• Serve as a patient advocate by coordinating care and ensuring access to necessary resources and support.
Qualifications:
Education:
• Graduate of an accredited School of Nursing required.
• Bachelor of Science in Nursing (BSN) preferred.
Experience:
• Minimum of four (4) years of nursing experience required.
• Two (2) years of recent acute hospital experience as an RN required.
• EPIC EHR experience required.
• Prior case management or utilization review experience preferred.
• No first-time travelers will be considered.
Licensure/Certification :
• Current Registered Nurse (RN) license in the state of North Carolina (compact license accepted if applicable).
• Basic Life Support (BLS) certification required (AHA only).
• ACM (Accredited Case Manager) or CCM (Certified Case Manager) certification preferred.
Skills &
Competencies:
• Strong clinical assessment and critical thinking abilities.
• In-depth knowledge of utilization management, discharge planning, and care coordination.
• Excellent communication and negotiation skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficiency in EPIC and Microsoft Office applications.
