Assistant Practitioner - Neuro Rehabilitation

Our team is a specialist integrated team of Occupational Therapists and Physiotherapists, working together to provide intervention for adults in the community setting covering the Liverpool footprint. • Assess, plan and implement individualised programmes of care within the skills and competencies expected of the role
• Work collaboratively within the team and with other providers to deliver local and national health priorities and objectives
• Deliver therapeutic interventions in line with national quality and standards and Trust policies.
• Actively support work colleagues and patients with compassion and courage acting as at their advocate and champion
• Be accountable for own clinical practice
• Work within the skills and competencies expected of the role
• Communicate effectively with patients and carers recognising the need for alternative methods of commination to overcome different levels of understanding, cultural background and preferred ways of communication
• Lead on health promotion
• Support clinical staff in managing long term conditions
• Participate in supervisions, development sessions, undertake all relevant mandatory training
• Participate in audits as required by the organisation
• Travel across the footprint of the organisation

We are looking for a Therapy Assistant Practitioner with a special interest and extensive experience in neuro rehabilitation to join our Liverpool Community Neuro Rehab Team.
