District Nursing Caseload Holder
Mersey Care NHS Foundation Trust – Band 6 – Part‑time
Responsibilities- Manage a defined caseload of housebound patients with complex needs.
- Provide proactive, responsive services through holistic assessments and clinical care for acute and chronic highly complex conditions.
- Ensure continuity of care and lead communication with identified stakeholders.
- Work within the 24/7 service model covering 8am‑8pm 7 days a week.
- Contribute to the organisation’s transformation programme to improve quality and efficiency.
Flexible working requests will be considered for all roles.
