Assistant Manager - Hospital Discharge Team

Role Overview

An Assistant Manager within Integrated Health and Social Care, part of the Contact Cares Hospital Discharge Service, will lead a dynamic team focused on early intervention, assessment, and care planning for adult patients to facilitate timely discharge from hospital.

• Manage and support a team of senior social workers and health & social care staff.
• Lead assessment and care‑planning activities for a wide range of adult patients.
• Facilitate collaboration with staff across health and social care to ensure seamless service delivery.
• Contribute to the continued development and transformation of the service.

• Qualified Assistant Team Manager / Advanced Practitioner / Social Worker.
• Full valid driving licence and access to a vehicle.
• Enhanced DBS Disclosure and Social Work England Registration required.
