We are seeking an enthusiastic and patient-focused Prescribing Pharmacist to join our busy and supportive GP practice with 7,000 patients. The successful candidate will play a key role in medicines optimisation, chronic disease management and preventative healthcare, delivering clinics for conditions such as Asthma, COPD, Diabetes, Hypertension and Women's Health, including HRT and contraception reviews. Working alongside our GPs, and wider multidisciplinary team, you will help improve patient outcomes through high-quality, evidence-based prescribing and medication management.
MainResponsibilities
- Undertake structured medication reviews and medicines reconciliation.
- Support safe, evidence-based and cost-effective prescribing.
- Manage repeat and acute prescribing systems and address prescribing queries.
- Provide expert medicines advice to clinicians, patients and carers.
- Lead on medicines optimisation initiatives and prescribing audits.
- Support chronic disease management, particularly:
- Asthma
- COPD
- Diabetes
- Cardiovascular disease
- Chronic pain management
- Women’s Health
- Monitor high-risk medications and undertake appropriate follow-up.
- Work collaboratively with community pharmacies, secondary care and other healthcare professionals.
- Contribute to quality improvement projects and achievement of practice and contractual objectives.

Salary:
Competitive, £52,845 to £67,762 pro rata, depending on experience, qualifications and prescribing responsibilities.
