Position: Case Manager, Housing Navigation- HYS-SDS

The Case Manager of Housing Navigation assesses, diverts, and navigates young adults and young families who are homeless to facilitate connection to King County’s Coordinated Entry System (CES). They utilize engagement techniques to assist young adults to successfully transition from the streets into housing.

FLSA Status: Non‑Exempt, hourly

Union Status: Union

Reports To: Manager of Outreach and Supportive Services

Essential Duties Administrative Tasks
  • Maintain complete and timely documentation of client files, including enrollment forms, goal plans, confidentiality, compliance related documents and case notes.
  • Compile monthly reports in compliance with agency and contract requirements.
  • Participate in weekly supervision meetings and monthly case management team meetings, case consultation meetings and team meetings.
  • Meet weekly with supervisor to discuss programmatic updates and current caseload.
Direct Service Tasks
  • Enroll young adults who are homeless into housing navigation as needed, carrying a maximum caseload of 35.
  • Provide outreach, navigation, and diversion services focused on housing placement and assist clients in developing and meeting their individual goals.
  • Assist in obtaining necessary documentation for housing, such as third‑party homelessness verification and identification, and securing additional financial assistance when needed.
  • Assist clients in navigating challenges related to the housing application process and respond to Coordinated Entry System (CES) requests.
  • Identify education resources and job training services.
  • Offer a series of training courses on interview preparation, career exploration, etc.
  • Assist with locating financial aid resources.
  • Assist with gaining access to ongoing healthcare.
  • Assist with obtaining access to transportation for attending appointments, job interviews, school, etc.
Case Management Tasks
  • Provide strength‑based case management services and ensure goal setting is client‑centered.
  • Conduct client intake and assessments.
  • Engage clients in the development of Independent Living Plans (ILPs).
  • Provide weekly individual case management sessions with clients.
  • Support clients in identifying housing resources and applying for housing that meets their individual needs.
  • Maintain accurate and updated client records and data on progress toward goals.
  • Complete quarterly service reports.
  • Provide follow‑up and aftercare services for clients upon program exit.
  • Participate in agency and professional group training and conferences.
  • Encourage clients to increase positive connections and expand personal support networks.
  • Collaborate with other employees to ensure clients are connected to appropriate internal and external referrals.
Advocacy
  • Develop referral network of specialized service providers.
  • Collaborate with other case managers, local government, school, and area employers to create jobs and mentoring opportunities for clients.
  • Work with community volunteers to develop new program resources and career options for clients.
  • Advocate on behalf of youth and young adults for system change.

We Require
  • Bachelor’s degree in a human‑service field preferred.
  • Minimum of 2 years of relevant working experience, including supporting youth and young adults experiencing homelessness.
  • Experience providing case management services with an emphasis on trauma‑informed, positive youth development, housing first and harm reduction.
  • Experience with motivational interviewing preferred.
  • Must satisfactorily pass criminal background check, including fingerprints.
  • Must satisfactorily pass tuberculosis (TB) test.
  • Must have a reliable vehicle, a valid Washington State driver’s license, auto liability insurance, and a safe driving record as defined by Friends of Youth.
  • Proof of COVID‑19 vaccination (or approved medical/religious exemption request).
