Accord Care Home Health, a licensed Medicare-certified agency, is seeking a customer service-driven and detail-oriented RN Clinical Manager to join our Maitland team!
This position is full-time, in-office (8:30 AM - 5:00 PM) and includes limited field case management as needed. The RN Clinical Manager is responsible for assessments, coordination of care, communication with referral sources, and supporting field clinicians to ensure the highest quality of patient care.
Responsibilities- Perform case management, oversight, and coordination of care for all skilled disciplines.
- Coordinate patient admissions with other members of the healthcare team.
- Provide nursing care and treatment of home care patients per plan of treatment and approved authorizations.
- Supervise all field staff on assigned cases.
- Current Florida RN License.
- Level II Background Screening and CPR Certification.
- Minimum 1 year of Medicare Home Health Case Management experience (required).
- Minimum 1 year of Medicare OASIS experience (required).
- Minimum 1 year of Medicare Home Health experience (required).
- Minimum 1 year of Home Health experience (required).
- Excellent time management and organizational skills.
- Strong communication and customer service abilities.
- Experience with Kinnser / Well Sky Medicare is a plus.
- 1 year Kinnser (preferred).
- 1 year OASIS (required).
- 1 year Home Health (required).
