Clinical Case Manager (Non-Skilled Home Care)
Location: Accord Care Home Health - Maitland, FL
Schedule: Monday - Friday | 8:30 AM - 5:00 PM
Salary: $70,000 - $80,000

We are seeking a Clinical Case Manager to join our Maitland, FL office and oversee non-skilled home care services
.
Manage and coordinate non-skilled home care cases
, ensuring clients receive exceptional service and support.
Serve as the main liaison between clients, caregivers, and referral sources.
Oversee daily office operations
, assisting with scheduling, authorizations, and client communications.
Conduct aide orientations
, trainings
, and supervisory visits to promote quality and consistency in care.
Monitor compliance with all Accord Care policies
, as well as state and federal regulations
.
Collaborate with leadership to continuously improve client satisfaction and caregiver performance.
- Current RN license in the State of Florida (required)
- 1+ year of experience in home care or community health preferred
- Excellent communication, leadership, and organizational skills
- Positive, professional attitude and a strong commitment to quality care
- Reliable transportation and valid driver's license for occasional field visits
- Familiarity with Alaya Care software a plus
Monday-Friday schedule | 8:30 AM - 5:00 PM (no weekends!)
