Position Summary
The Nurse Practitioner (NP) is an integral member of the renal health care team in the provision of comprehensive health care services. Responsibilities of the role include coordinating the plan of care for a renal patient population through modality transitions with a focus on independent dialysis education. Facilitate and/or provide independent dialysis mentorship to various groups, including staff in the Regional Renal Program, hospital, long‑term care facilities and community care providers (regulated and non‑regulated) and/or medical residents, in order to promote and encourage the use of independent dialysis modalities for appropriate patients.
Emphasis will be on supporting an environment for collaborative health management with the patient, family, community partners, and renal program. Autonomous practice is enhanced by the use of established medical directives (as required) and consultation with renal physicians. The NP will provide collaborative care related to outpatient in‑facility chronic dialysis patients with a close link to the patient’s primary care practitioner for non‑dialysis health issues.
The NP will lead comprehensive person‑centered care initiatives for patients with chronic kidney disease (CKD) along the continuum of their nephrology care; perform a comprehensive medical/nursing history, physical and psychosocial assessment; demonstrate advanced health assessment and clinical decision making for complex renal patients; develop effective partnerships with primary care practitioners and other members of the inter‑professional team that support CKD patients to reach their optimal level of health, wellness and independence;
support CKD patients in their selection of independent dialysis (PD and HHD); provide home visits as required to support CKD and home dialysis patients; provide a key leadership role in nursing, including disseminating knowledge and best practice advancement. Evaluating the quality of care and patient outcomes within the renal patient population, in partnership with the physicians and the inter‑professional team;
identify quality initiatives within the renal program that will strengthen and further develop nursing professional practice and contribute to others understanding and application of research and evidence‑based practice.
The NP will follow the College of Nurses standards appropriate to the role. Nephrology experience is required in order to work onsite, and satellite locations. The successful candidate will also participate in the ongoing development, implementation and evaluation of the role.
