We are looking for a compassionate Registered Practical Nurse to join our Community Support Services team at Riverside Health Care. This full-time role demands strong clinical skills and effective communication to deliver high-quality nursing care. You will be coordinating clinical interventions and collaborating with an interdisciplinary team to ensure client well-being.
Key Responsibilities:
• Deliver individualized nursing care to clients
• Coordinate and implement clinical interventions
• Assess and monitor changes in client conditions
• Facilitate care transitions with clients and families
• Maintain client confidentiality and professional standards
Requirements:
• Graduate of an accredited nursing program
• Current CNO registration in good standing
• Valid Ontario Driver's License and personal vehicle
• Basic Life Support certification
• Excellent interpersonal and communication skills
