The Nurse Practitioner will be a vital member of our renal care team, coordinating comprehensive health services and providing mentorship to medical staff. You will facilitate independent dialysis education and ensure collaborative patient care. Your mission is to empower patients on their healthcare journey and enhance their quality of life through innovative health management.
Key Responsibilities:
• Manage care plans for chronic kidney disease patients
• Teach independent dialysis modalities to staff
• Assess physical and psychosocial health comprehensively
• Foster partnerships with healthcare professionals
• Implement initiatives to improve patient care
Requirements:
• Master’s NP degree or 5 years hemodialysis experience
• Current RN (EC) registration in Ontario
• BCLS certification
• Strong leadership and teaching capabilities
• Proven track record of teamwork and critical thinking
