* Host and provide care through community-based medical clinics
* Deliver primary healthcare services to individuals across the community
* Conduct home visits when required
* Lead vaccination and immunization clinics
* Collaborate with interdisciplinary teams and community partners
* Coordinate care and liaise with primary care and healthcare resources
* Maintain strict privacy, confidentiality, and professional standards
* Provide compassionate, trauma-informed care with empathy and respect
* Support individuals experiencing barriers to healthcare access

* Completion of a recognized Primary Care Nurse Practitioner Program
* Bachelor of Science in Nursing (BScN)
* Current registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario
* A valid Ontario driver's licence, reliable vehicle, and automobile insurance
* Experience in primary care and/or community health is considered an asset
* Understanding of the social determinants of health and the impact of social isolation on individual and community well-being
* Excellent interpersonal, communication, and conflict resolution skills
* Strong emotional intelligence and the ability to establish healthy professional boundaries
* Excellent verbal and written English communication skills; additional languages are an asset
* Proficiency with Microsoft Office, email, and other computer applications
