This Nurse Practitioner role supports a diverse community, spanning all life stages. You will deliver primary care services, emphasizing health promotion and disease prevention. Key Responsibilities:

• Provide comprehensive primary care services across diverse populations

• Promote health and educate patients on disease prevention

• Develop and evaluate primary care programs and services

• Collaborate within a multidisciplinary team

• Utilize electronic medical records and maintain documentation

Requirements:

• Baccalaureate in Nursing and Master’s in Nursing

• Current RN(EC) registration with College of Nurses of Ontario

• Demonstrated experience in primary care or community settings

• Knowledge of health promotion and risk reduction practices

• Strong communication and multitasking skills
