We are currently recruiting for Registered Practical Nurse to provide services to MD Care Family Health Organization. While these positions are employed by Community Health caring Kitchener-Waterloo, the RPN’s day-to-day work location (across 3 locations in 5km radius), schedule of hours, as well as applicable policies and procedures, will be determined by MD Care Family Health Organization Team to which they are assigned.

Practical Nurse – MD Care Family Health Organization

$27.95 -$33.68 per hour

Full Time or Part Time, Permanent

Up to 37.5 Hours per week – Open to 1 RPN or 2 Part-time RPN’s

Position Summary

The Registered Practical Nurse (RPN) provides comprehensive, patient-centred primary care within MD Care Family Health Organization supporting patients. Working to full scope of practice as part of an interdisciplinary team, the RPN delivers direct clinical care including assessments, vital signs, immunizations, chronic disease support, and health education. The RPN collaborates with physicians and other health professionals to improve and support better health outcomes for individuals, and families.

Responsibilities
  • Complete history of clients, anticipate and ensure equipment for client visit is set up in room.
  • Triage client’s needs, either by telephone or in person.
  • Screen and assess clients with minor episodic illnesses according to established protocols.
  • Perform basic nursing skills (e.g., dressing of wounds, injections, suture /clip removal and assisting with other procedures as requested).
  • Provides Health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Practical Nurse and all relevant provincial legislation and regulations.
  • Perform necessary clinical functions to facilitate delivery of comprehensive primary care and patient flow (e.g., assist providers with prescription ordering; clean equipment).
  • Maintain accurate health records and up to date clinical notes in accordance with College of Nurses Standards and policies.
  • Ensure all exam rooms are stocked sufficiently and consistently.
  • Order medical supplies and maintain inventory controls.
  • Maintain contact with pharmaceutical company representatives and maintain a current inventory of pharmaceutical samples.
  • Organize printed resources such as client education materials, diagnostic and other requisitions.
  • Develop and monitor operating budgets for assigned projects/programs and ensure expenditures remain within budget.
  • Maintain a valid driver’s license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with the Health Centre’s policies.
  • Work during both regular and extended hours (evenings/weekends) as determined by the Health Centre & MD Care Family Health Organization. A minimum of one evening per week will be required.
  • Participate in team and staff meetings and other meetings.
  • Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  • Work in a manner that preserves privacy and confidentiality.
  • Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  • Work in a manner that demonstrates self-reflection and personal accountability for work performance.
  • Collect, analyze and report on data and relevant information as required.
  • Participate in research as determined by the Health Centre.
  • Adhere to all Health Centre and MD Care Family Health Organization policies.
