The Wellness and Care Planning Nurse role supports the Director of Wellness in maintaining resident wellness goals, health records, and compliance, while handling medication administration, charge staff duties, and coordination with Ontario Health at Home, physicians, and community services.

The schedule for this role is 3 days per  this role, you would be:
  • Assist Director of Wellness in creating and maintaining resident wellness goals
  • Maintain updated health information for residents
  • Support wellness team in DOWabsence
  • Coordinate with Ontario Health at Home, community services, physicians & facilitate in-house physician visits
  • Work with Director of Wellness in efforts to maintain compliance
  • Support team members with care planning execution
  • Champion Health and Safety practices within the home
  • Other duties and responsibilities as assigned
We are looking for a candidate with the following qualifications:
  • RPN or higher designation, with current registration with the College of Nurses
  • Current First Aide and CPR certifications
  • 3-5 years of related nursing experience
  • Experience in supervision of staff required
  • 3-5 year experience in retirement home setting an asset
  • Ability to multi-task, prioritize and manage time effectively
  • Working Knowledge of RHRA an asset
  • Demonstrated ability to participated and function in a team orientated environment
  • Excellent knowledge of MS office, EMAR, Med Ecare and related software
  • Superior communication, presentation and interpersonal skills
  • WHMIS/CPR Certified
  • Committed to resident centred care, and customer/hospitality service

This is a temporary role.
