This part-time position involves working with a dedicated team committed to improving Residents' physical and emotional health. You will be responsible for assessing care needs, managing medication, and engaging actively with families to create individualized care plans. Your work will deeply influence our community, supporting both residents and their loved ones.

Key Responsibilities:

• Assess residents' needs collaboratively

• Lead the PSW team in delivering exceptional care

• Track and administer resident medications

• Communicate effectively with families about care plans

• Document changes in resident conditions accurately

Requirements:

• Active CNO registration

• VSS check within the past 6 months

• Certifications in First Aid and CPR
