In this position, your contributions will encompass comprehensive patient assessments, vital signs monitoring, chronic disease support, and immunization delivery. Working within an interdisciplinary team, you will help address health inequities and improve the lives of individuals and families in the region. Key Responsibilities:

• Triage and address client health concerns

• Conduct thorough patient assessments and maintain records

• Assist with immunizations and health education

• Order medical supplies and manage inventory

• Coordinate care with health professionals efficiently

Requirements:

• Valid registration as a Registered Practical Nurse

• At least 1-2 years of nursing experience

• Familiarity with community health resources

• Effective communication skills, both written and verbal

• Ability to work independently and in a team
