As a Community Nurse with Care Partners, you will play a crucial role in providing exceptional nursing services within Ontario's healthcare framework. Flexibly working during school hours, you’ll develop care plans and collaborate with both patients and healthcare teams. Key Responsibilities:
• Create and manage individualized care plans
• Deliver nursing services in a clinic setting

Requirements:
• RN or RPN degree or diploma
• Registration with the College of Nurses of Ontario
• Strong commitment to patient care excellence
• Exceptional communication skills in English
• Valid CPR certification and current immunization
