Requirements

A graduate of an accredited program in Social Work and are a member in good standing of Ontario college of Social Workers and Social Service Workers

Recent experience in managing patients in acute/rehab or community‑based rehabilitation programs is preferred

Reliable, flexible, creative, self‑directed professional, able to work effectively and collaboratively with a high‑performance interdisciplinary rehabilitation team

Sound knowledge of community resources is an asset

Effective interpersonal, organisational and communication skills

A positive work and attendance record is essential

Ability to adapt to new computer applications

This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community‑based rehabilitation setting
