Traverse Independence is seeking compassionate part-time Community Facilitators to support individuals with acquired brain injuries in the Region of Waterloo. You'll provide vital assistance in homes and community settings, working collaboratively with a multidisciplinary team. Key skills for success include excellent communication, organizational ability, and a dedication to enhancing clients' independence and quality of life.
Key Responsibilities:
• Build trusted relationships with clients to support independence
• Develop and evaluate individualized client programming
• Assess client needs and create supportive strategies
• Liaise with community resources and professionals
• Report to a Service Coordinator and collaborate with team members
Requirements:
• Community College Diploma in Social Services or related field
• Proven organizational and communication skills
• Ability to work in a multidisciplinary environment
• Valid driver's license and reliable vehicle required
• Experience with ABI preferred, training provided
