Key Responsibilities:

• Build supportive connections with clients in various settings

• Develop and assess individualized client program plans

• Help clients navigate community resources and supports

• Collaborate effectively with multidisciplinary teams

• Document progress and client needs accurately

Requirements:

• Diploma in Social Services or related field preferred

• Strong interpersonal and organizational skills required

• Familiarity with ABI beneficial; training provided

• Valid driver’s license and access to a car with insurance

• Proficient in Microsoft Office applications
