About the job:
The Community Support Services (CSS) program, offered through the Healing and Wellness Branch of the Métis Nation of Ontario (MNO), is designed to help MNO Citizens maintain their independence and quality of life. The program supports individuals living with chronic illnesses, those who are terminally ill, and frail or elderly individuals who may be isolated
At the heart of the CSS program is the coordination and delivery of transportation for medical needs. Additional supports include friendly visiting, caregiver support, advocacy, client referrals, home safety assessments, and assistance with navigating complex systems such as Ontario Works and the Ontario Disability Support Program (ODSP). For clients with literacy challenges, coordinators may provide reading support in accessing resources for end-of-life care.
Volunteers are an essential part of the CSS program. Coordinators are responsible for recruiting volunteers, assigning services, and managing related payments and documentation.
CSS Coordinators also play a vital role in building and sustaining strong relationships with local partners and service providers. These collaborations enhance service delivery, promote Métis culture and identity, and support the overall wellness of MNO Citizens. Through ongoing community engagement, the CSS program fosters inclusive, culturally relevant supports that reflect and honor the unique experiences of Métis people.
What you will be doing:
Lead clients in a supportive process to determine clear wellness plans based on immediate need
Assess, determine and facilitate economic use of funds related to providing medical transportation and accommodations while balancing client need
Attend medical appointments and consultations with client
Provide education/information related to health and well-being and activities to reduce isolation through workshops or group-based activities
Recruiting volunteers, assigning services, and managing related payments and documentation
Annually obtain copy of insurance and valid driver license
Coordinate Volunteer Medical Transports and/or Friendly Visits Schedule
Evaluate Volunteers with support and direction from Supervisor
Organize an annual Volunteer Recognition Event
Maintaining documentation of client services in the Online Health and Access Tracking System (OHATS), Data basics and Excel tracking documents
Financial reporting specific to local program site budget allocations and parameters
Reflect Métis cultural approaches as a part of the activities and services
Participation in MNO Healing and Wellness training as needed/determined
