Lead mental health initiatives as a Case Manager in Seaforth, supporting individuals living with serious mental illness. Offer tailored case management services tailored to client needs.
As a Case Manager, you will play a critical role reporting to the Program Manager. You'll provide client-centered support, engage in crisis management, and develop recovery plans in a variety of community settings. Your contributions extend to educational outreach within the community, representing our values and mission with integrity.
Key Responsibilities:
• Provide comprehensive case management and advocacy
• Develop recovery goals with clients for stability
• Perform assessments in-home and coordinate with local services
• Prepare and facilitate comprehensive referrals
• Assist in community education and awareness efforts
Requirements:
• Bachelor’s Degree in Social Work or related field preferred
• 1-3 years of relevant experience in mental health
• Strong knowledge of addiction resources and recovery practices
• Excellent communication and interpersonal skills
• Ability to adapt in diverse situations
