Qualifications
  • Required:
    • Current Oregon licensure as a Physical Therapist or eligibility and evidence of application for licensure.
    • Pelvic floor physical therapists: completed at least level one training through a recognized training institution.
    • Computer literacy skills.

Typical Physical/Mental Demands

Able to stand, walk, bend, squat, and reach frequently. Possesses physical agility and adequate reaction time to respond quickly to unexpected patient care needs. Must be able to see, hear, and speak with or without assistive devices. Must lift 50 pounds during patient mobility occasionally and 25 pounds or less frequently using proper body mechanics. Communicate effectively through reading, writing, and speaking in person or on telephone.

Proficient in executive function skills: planning, organization, focusing attention, flexible thinking/problem solving, memory, self‑monitoring, and juggling multiple tasks.

Essential Job Functions
  • Under a valid referral process, carry out an individualized program of physical therapy, including assessment, treatment, planning, implementation, education, and effective communication to maximize a patient’s progress toward functional goals.
  • Provide direct patient care within scope of practice. Seek excellence by utilizing evidence‑based or best‑practice techniques.
  • Know and apply skills to care for the physical and developmental needs of the population age groups receiving services. Be kind and welcoming to all, regardless of gender, ethnicity, race, sexual orientation, socioeconomic status, or other factors.
  • Provide counseling to patients and families/caregivers about results of testing and recommended treatment options. Involve patients and families in goal setting and treatment decision‑making.
  • Appropriate supervision of Physical Therapy Assistants, with regular and timely communication.
  • Appropriate use of Rehabilitation Aides.
  • Continuously monitor patients’ functional needs and progress, adjusting plan of care as needed to ensure optimal outcomes.
  • Clear, concise, and timely documentation: evaluation, plan of care, daily treatments, progress notes, and discharge summaries.
  • Clear and timely documentation of other conversations with or regarding patients.
  • Demonstrate integrity through compliance with insurance regulations, accountable billing procedures, judicious use of patient’s resources, and HIPAA rules.
  • Maintain good efficiency through effective use of downtime from patient no-show or cancellation events.
  • Comply with hospital and departmental policies, quality improvement program, safety, environmental and infection control standards.
  • Support ancillary staff: clear communication with front‑office staff and compliance with department procedures for scheduling, cancellations, no‑shows, and other front‑office duties.
  • Encourage innovation by enhancing professional growth and development of self and others through participation in educational programs, in‑services, and workshops. Keep up to date on professional literature.
  • Communicate in a kind and respectful manner to maintain positive relationships with all health professionals, ancillary staff, patients, and caregivers.
  • Ability to remain calm in emergency or crisis situations.
Marginal Job Functions
  • Serve as Clinical Instructor for Physical Therapy students at various stages of education.
  • Observe and mentor high‑school or undergraduate students.
  • Promote the profession and Sky Lakes organization through participation in in‑services to residents or other hospital departments, and community education events.
  • Perform other duties as assigned.
