PRNPer Diem – As Needed)
Shift: 5:00 pm – 5:30 am

• Respond to resident needs promptly and kindly (50%)
• Read and follow care plans (10%)
• Assist with bathing, dressing, hygiene & bowel & bladder functions (20%)
• Support mealtime service and clearing (3%)
• Transport residents safely and escort to meals & activities (3%)
• Perform safety checks and encourage resident activities (1%)
• Chart and document information (4%)
• Respond to emergency alerts and assist in safety drills (3%)
• Ensure resident safety and comfort at all times (1%)

• Maintain a positive professional image
• Communicate effectively with coworkers and departments
• Manage time to complete resident assignments each shift
• Adhere to meal break and rest period policies
• Perform other duties consistent with the position

• Age 18 or over with valid work authorization
• High school diploma or equivalent
• Preferred:
  Nurse‑Aid certification, First Aid certification
• Background clearance as required by law
• Negative TB test (within 6 months) and health screening
• Prior caregiving experience preferred; on‑job training available
• Fluency in English, both speaking and written
• Ability to follow oral and written directions
• Dedicated to excellent patient interactions
• Met all state regulatory requirements

• Mobile, able to lift 40+ lbs unassisted and push 150 lbs
• Capable of kneeling, squatting, overhead reaching, and bending
