Recovery Support Coordinator at Groups Recover Together

Groups is a leading outpatient provider specializing in substance use disorder treatment. We are committed to supporting underserved communities hit hardest by the opioid crisis. Since 2014, our local care teams have guided hundreds of thousands of individuals on their path to recovery, helping them reclaim their purpose and dignity through compassionate, collaborative care.

Our evidence‑based approach combines medication, group therapy, and personalized support, delivered online and in person by local providers. We believe recovery extends beyond the traditional office visit, helping members build a foundation for long‑term success and the fulfilling lives they want and deserve.

Responsibilities
  • Provide reasonable accommodations to enable individuals with disabilities to perform essential functions of this position.
  • Collaborate with a multidisciplinary care team—including counselors, prescribers, and cross‑functional members—to ensure high‑quality, member‑centered care; participate in regular interdisciplinary case conferences to drive decision making.
  • Build trusted relationships with members to facilitate change and help them achieve recovery goals through empathy, motivational interviewing, and lived experience when applicable.
  • Assist members in setting personal and treatment‑related goals, providing individual coaching, coping‑skill support, and engagement in recovery activities.
  • Support unengaged members to reengage in treatment services through intensive outreach interventions.
  • Introduce new members to the Groups care model and support them through transition to care via outreach and engagement efforts.
  • Provide in‑person, virtual, and telephonic support based on member and regional needs.
  • Maintain a shared caseload and collaborate with the care team to eliminate barriers to care.
  • Support completion of care pathways for members in special populations (e.g., high‑risk of disengagement or poor outcome).
  • Connect members to healthcare, behavioral health, social service, and community‑based resources; follow up to confirm needs are met.
  • Help members navigate systems such as insurance, pharmacy, housing, and transportation, empowering them to advocate for their own needs.
  • Manage referrals, releases of information, and other documentation to facilitate smooth transitions.
  • Act as a community connector and advocate, promoting recovery‑positive language, reducing stigma, and fostering inclusion; become an expert in local community resources and recovery networks.
Administrative & Operational Support
  • Deliver outstanding customer service to members, visitors, and community partners.
  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
  • Support daily operations, including group facilitation logistics, scheduling assistance, office maintenance, and safety standards.
  • Assist clinicians and members with treatment‑related tasks such as prescription management, toxicology submissions, appointment scheduling, and responding to inquiries via phone, SMS, email, chat, and other channels.
  • Participate in weekly interdisciplinary team meetings and ongoing continuing education as required.

  • Ability to identify, navigate, and connect members to appropriate community resources and support systems.
  • Excellent organizational, time‑management, and problem‑solving skills, handling multiple priorities.
  • Ability to maintain confidentiality, professional boundaries, and high ethical standards.
  • Proficiency in G‑Suite, email, electronic medical records, and digital tools.
  • Strong problem‑solving skills and adaptability in a team‑based environment.
