Licensed Practical Nurse (LPN) Position Summary
The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient's needs.
Essential Functions- Administer medications safely, accurately, and based on patient age and clinical condition.
- Minimize potential for medication errors by following proper medication administration procedures.
- Assist the RN with initial assessments and patient reassessments per policy.
- Identify and implement appropriate rehabilitation nursing interventions.
- Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
- Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
- Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
- Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
- Develop and implement individualized teaching plans based on patient and family learning needs.
- Perform other duties as assigned.

Ability to work effectively in an interdisciplinary team environment.
Knowledge, Skills & AbilitiesThe requirements listed below are representative of the knowledge, skills and/or abilities required.
Education: Graduate of an accredited Licensed Practical/Vocational Nurse program.
Experience: Minimum six months of Medical/Surgical experience in an acute care setting preferred.
License: Current state licensure as a Licensed Practical/Vocational Nurse.
Certifications: CPR/BLS certification required; IV Certification highly preferred or must be obtained within 6 months.
