The Knoxville-Knox County Community Action Committee (CAC), in partnership with Knoxville’s Community Development Corporation (KCDC), is hiring a Case Manager to serve residents of KCDC high-rise properties. You will work with elderly and disabled individuals who are at risk of eviction in this full-time, 37.5 hour/week position. You will provide on-site, intensive, client-centered case management that promotes housing stability and prevents homelessness — helping residents understand and comply with lease requirements, address behavioral or health-related concerns that threaten their tenancy, and access the income, benefits, and community resources that support long-term stability and quality of life.
You will be based in a dedicated office inside a high-rise property and work in close partnership with property management staff. You will meet, regularly and as needed, with case-managed residents by appointment and on a walk-in basis as capacity allows. Hours of operation will be weekdays, generally 8:30 a.m. to 4:45 p.m., with flexibility for off-site trainings, meetings, and community obligations.
You will report to the Homeward Bound Director.
- Assess housing stability, income, health, mental health, substance use, and support systems; develop and monitor a measurable case plan with each resident.
- Explain leases and KCDC policies one-on-one — lease terms, grounds for violations and eviction, and rights and responsibilities under HUD guidelines — and coach residents on habits that sustain compliance.
- Advocate and mediate with property management around conflicts, behavioral concerns, housekeeping issues, and lease misunderstandings.
- Screen for and assist with applications and recertifications for Social Security/SSI/SSDI, Veterans benefits, SNAP, and other public benefits; support budgeting, repayment agreements for back rent or utilities, and connections to emergency financial assistance.
- Explore employment and supported-employment options, including workforce referrals, job training, resumes, applications, and interview preparation.
- Coordinate with healthcare providers, clinics, and mental health agencies to link residents to primary and specialty care, mental health services, counseling, psychiatric services, and substance use treatment; support medication management and appointment adherence.
- Respond quickly to crisis referrals — conference letters, Notices to Vacate, and significant housekeeping or safety concerns — de-escalating situations and building short-term safety or compliance plans to prevent loss of housing.
- Assist residents with 10-day hearings, refer to Legal Aid and other legal resources, and accompany residents to conferences or court as a support person and informational resource (not as legal counsel).
- Explore alternative housing when staying in the current unit is no longer appropriate, supporting transitions that avoid formal eviction and minimize barriers to future housing.
- Provide direct assistance such as escorted transportation, bill paying, form completion, and appointment scheduling; maintain client files and record services in the Homeless Management Information System (HMIS).
- Meet with property managers at least twice monthly to review referrals, discuss trends, and jointly plan interventions.
- Knowledge of issues affecting the chronically homeless and people with disabilities, including mental illness and substance use; knowledge of basic counseling techniques and community social services.
- Computer literate, with the ability to use word processing software, access databases, and record activities; able to produce accurate reports and correspondence.
- Valid Tennessee driver’s license, reliable automobile for…
