Nurse Practitioner/Physician Assistant Primary Care

Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house Support Staff.

Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.

• Review and sign MARs as requested by facilities; sign all primary‑care orders at the facility or within 24 hours if faxed or emailed, including phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders as appropriate, and other relevant orders; review and address labs within 24 hours of receipt; manage narcotic prescriptions per the Eventus Whole Health Narcotic Rx Policy;
  
  provide sufficient time each visit for paperwork and administrative duties.
• Complete documentation and submit billing records to Eventus Whole Health according to the published charting and bill‑sheet policies, and provide the facility with a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record.
• Establish and maintain open and positive communication with facility staff and administration; provide verbal and/or written instruction or feedback regarding medications and other caregiver information; meet with the administrator or designated facility care coordinator prior to the start and prior to leaving the facility each day.
• Be knowledgeable of and adhere to Eventus Whole Health standards, policies and procedures; be aware of and comply with all legal, regulatory and ethical guidelines.
• Maintain patient confidentiality at all times, including appropriate use of cell phone, emails, text messaging, patient charts and EMR; agree to abide by HIPAA rules and regulations.
• Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding a resident’s care.

For Nurse Practitioner:

• Master’s degree in Nursing from an accredited institution and a current, valid license to practice medicine issued by the state in which you reside. All other necessary certifications must be current and valid.

OR

For Physician Assistant:

• Graduation from an accredited physician assistant program (National Commission on Accreditation of Allied Education Programs).
• Passing of the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
• License from the state in which you reside as a Physician Assistant.
• Specialization in geriatrics, family, or psychiatry desired.
