Nurse Practitioner – Geriatric Primary Care
Summary: An innovative and mission-driven healthcare organization is seeking a Nurse Practitioner (NP) to join its brand-new PACE (Program of All-Inclusive Care for the Elderly) center in Kokomo, IN
. This role is perfect for an NP with a passion for geriatric care, team collaboration, and building new care models from the ground up. The center supports seniors who wish to age in place
, offering coordinated medical and supportive services in a vibrant, community-based setting.
- Serve as a core member of the interdisciplinary team (IDT), working closely with social workers, PT/OT, dietitians, and other care professionals
- Conduct comprehensive health histories and physicals for new participants
- Manage and update individualized care plans, providing ongoing primary care
- Provide preventive care
, immunizations, screenings, and medication management - Make referrals to specialists as needed and coordinate external care
- Deliver care across multiple settings
: in‑center, in participants’ homes, and in contracted facilities (nursing homes, hospitals, etc.) - Collaborate with the Medical Director and other providers to ensure high‑quality outcomes
- Support the launch of the new center by helping develop workflows, processes, and team structure
- Travel up to 50% locally as the participant census grows
- Graduate of an accredited Nurse Practitioner program
- Certified as a Family, Adult, or Geriatric NP (AANP or ANCC)
- Active NP license in Indiana
- Current DEA registration and BLS certification (AHA or Red Cross)
- 2+ years of NP experience
, including 1+ year working with frail/elderly populations in primary care or long‑term care - Strong understanding of value‑based care and Medicare/Medicaid models
- Experience with team‑based care and collaborative care planning
- Excitement for building and growing a new care center
- Leadership or program development experience
- Knowledge of PACE or similar elder care programs
- Comfort with EMRs and tech tools for mobile documentation
- Competitive salary: $125,000–$150,000 (based on experience)
- Flexible schedule options (4x10s or 5x8s)
