• Lead with Heart:
  Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences
• Be the Advocate:
  Champion each patient's unique needs, preferences, and wishes while supporting their loved ones
• Create Meaningful Connections:
  Build relationships that matter, providing counseling and emotional support when it's needed most
• Make Every Day Count:
  Develop and implement care plans that prioritize quality of life and comfort
• Tell Their Story:
  Document each patient's journey with care, capturing progress and precious moments

Your Impact Zone:
As an RN Case Manager, you'll travel throughout the Wabash and Kokomo areas, bringing healing and hope directly to patients in their homes, assisted living facilities, long‑term care facilities, and hospitals – wherever they feel most comfortable.

• Active RN License
• Valid Driver's License
• 1+ years hospice experience

• BLS Certification
• Hospice and Palliative Medicine background
