Social Worker, Hospice & Palliative Care Full-Time Central Wisconsin

A hospital near La Crosse, WI is seeking a compassionate Social Worker to join their Hospice & Palliative Care team!

Social Worker, Hospice & Palliative Care Care Schedule:

• Full-Time Direct Hire
• Monday Friday 8:00 am 4:00 pm

Social Worker, Hospice & Palliative Care Responsibilities:

• Provide psychosocial assessments, counseling, and support for patients and families
• Collaborate with an interdisciplinary team to develop and update individualized care plans
• Educate patients, caregivers, and families on palliative care, comfort measures, and the dying process
• Maintain accurate documentation, uphold ethical/legal standards, and participate in team meetings and quality initiatives

Social Worker, Hospice & Palliative Care Qualifications:

• MSW from an accredited School of Social Work
• Wisconsin Certificate from the Department of Regulations & Licensing Examination Board of Family Support Coordinators
• Minimum of 3 years of clinical experience
• Completion (or agreement to complete) hospice/volunteer training
