What You'll DoAs a Community Health Worker, you will play a direct role in providing LOVE + MEDICINE for a broad range of patient populations within our Level II ACS-verified Trauma Center and Emergency Services department as well as our newly created EmPATH behavioral health unit. You will be expected to perform with a high degree of professional aptitude while helping deliver patient care and resources as part of a multidisciplinary team of Providers, RNs, Paramedics, EMTs, Respiratory Therapists, Care Coordinators, Health Unit Coordinators, and Social Workers.
You will work in partnership with our Emergency Services/EmPATH teams and community programs/services/resources to promote health equity and overcome challenges. You will focus on fostering and developing patient relationships and engagement to discover and recognize social determinants of health disparities to achieve population health initiatives. You will work to identify community level barriers to cultivate additional partnerships to bridge gaps for the targeted population while advocating for patient needs, perform outreach, help patients navigate resources and access community services, provide patient and community centered education, and identifying opportunities to enhance care coordination.
Key responsibilities include:
- Develop trusted relationships and connections with community and health care resources, services, and partners
- Maintain accurate health data on identified patient population(s) and develop outcome measures
- Provide expertise in community-based resources for medically complex patients
- Assess social determinants of health screenings and questionnaires
- Identify and/or develop community resource packets and culturally sensitive patient education materials for educating patients and caregivers about relevant topics, programs, and resources (appropriate utilization and access to health care services)
- Coordinate communication and follow-up with patients, providers, community entities, etc., in accordance with plan of care (telephonic, virtual, face-to-face, or home visits)
- Meet with patients and families to assist with paperwork, forms, and resource materials
- Contact insurance companies to verify benefits to ensure referrals are covered
- Contact pharmacies when needed in conjunction with patients to help advocate for patient
- Practice and promote behavior consistent with our Mission, Vision, and Values
What s Available
:
- Fulltime, 80 hours biweekly (1.0 FTE)
- Variable day shifts, Monday-Friday, typically between the hours of 8a-5p
- Onsite in La Crosse, WI with travel within the community based on patient needs
- Starting pay of $18.63 per hour and up, based on your years of experience
- High School Diploma or equivalency required
- Valid Driver's License (DL) required
- Minimum one year of experience in health care or human service-related field, familiarity with community resources
- Excellent communication skills, both written and verbal, with the ability to effectively communicate complex information to diverse audiences and foster positive working relationships and promote collaborative practice
- C3 Training Program through AHEC, HUB or Apprentice Program required within one year of hire date
- Prior Emergency Services experience desired
