Eagle Crest Communities is seeking Nurses to join the team of healthcare professionals at our skilled nursing and rehab facility, Riverside Transitional Care. These individuals will bring their high‑quality clinical skills and can‑do attitude to the team. The successful candidates can be either a Licensed Practical Nurse or Registered Nurse.

Shift Availability:

• Full‑Time: 80 hours per 2‑week pay period
• Casual Call:
  Build your own schedule – 3 shifts per month, 2 on the weekend
• Part‑Time: 16+ hours per 2‑week pay period
• Available shifts: days of the week may vary and will include varied weekends and holidays. These positions enjoy every other weekend and every other holiday off!
  AM: 6:00‑2:30 pm / 6:00‑6:30 pm / 8 or 12‑hour shift options available
  PM: 2:00‑10:30 pm
  NOC: 10:00‑6:30 am / 6:00‑6:30 am / 8 or 12‑hour shift options available

• Perform basic nursing care and ensure all safety measures and body mechanics are followed in the delivery of care.
• Document and report all care given and relevant observations regarding residents to the appropriate staff person.
• Uphold the Mission Statement of Bethany Lutheran Homes, Inc., and perform all duties in a manner consistent with its Core Values.
• Provide nursing care and assistance to rehab, geriatric, and/or memory care populations.
• Coordinate, implement and evaluate nursing care.
• Participate in the planning of resident care.
• Provide treatments and administer medications.
• Assist with activities of daily living as needed.
• Provide guidance and leadership to Nurse Technicians, Certified Nursing Assistants, Certified Medication Assistants, and Support Aids.

• Current license to practice in Wisconsin as a Licensed Practical Nurse or Registered Nurse.
• Current BLS CPR Certification.
• Expertise in chronic geriatric disease is a plus; we will train the right fit for our team.
• Previous experience working in long‑term care or acute care (preferred).

• LPN – Starting Wage: $30.50+ per hour.
• RN – Starting Wage: $39.25+ per hour.
• Pay based on experience and qualifications.
• We pay for high performance! Wage review at six months.
• Premium Pay if opting for every weekend position.
• PM Differential, increased NOC, and weekend differentials apply.
